The Vermont Compliance Officer Form should be apart of the compliance strategies of life sciences organizations conducting business in the state. Both pharmaceutical and medical device manufacturers should file their Compliance Officer Form with the State of Vermont by Jan. 1, 2019 for the 2018 filing. Manufacturers with expenditures above $0 for the previous reporting period must complete the form and send a registration fee of $500 by the first day of each reporting period.

Manufacturers that complete the form and send in their $500 fee and later determine they have no reportable expenditures for 2018 may request a refund. Refunds of registration fees will not be reviewed for approval and processing until 60 days after the reporting deadline.
