Vermont Compliance Officer Form Due January 1st

Pharmaceutical and medical device manufacturers should file their Compliance Officer Form with the State of Vermont by January 1st 2020 for the 2019 filing. Manufacturers with expenditures above $0 for the previous reporting period must complete the form and send a registration fee of $500 by the first day of each reporting period.

The Compliance Officer Form can be completed on the Vermont Attorney General’s website by using the following URL: http://ago.vermont.gov/compliance-officer-form/

Manufacturers that complete send in their $500 fee and later determine they have no reportable expenditures for 2019 may request a refund. Refunds of registration fees will not be reviewed for approval and processing until 60 days after the reporting deadline. To request a refund, complete the refund form available at http://ago.vermont.gov/prescribed-products-refund-form/
